As we begin our work together, there’s quite a few formalities that need to be taken care of up front, primarily meaning tons of forms!

(I know it, I hate paperwork too but I promise once this part is done we can get on to the best part–helping you feel better & working through your current challenges).

To expedite the process, you’re more than welcome to print these forms, complete them in advance, and bring them with you to your first appointment.
